We come to you—in your home or online via secure telehealth—to complete a comprehensive, one-on-one evaluation. Our goal is to understand your strengths, challenges, daily routines, and priorities so we can design a plan that’s truly individualized.
What’s Included
Clinical Interview & Intake Review
We start with your Intake Form (medical history, concerns, goals, daily activities) and a guided conversation with you and, if you wish, a family member/caregiver.Standardized & Functional Testing
Targeted assessments of speech, language, cognition (memory, attention, problem-solving, executive function), voice, and swallowing—combined with real-life tasks (e.g., medication reminders, scheduling, mealtime strategies).Observation in Your Environment
In-home evaluations let us see the real setup—lighting, seating, meal routines, technology—so recommendations fit your life. (Telehealth visits use virtual walk-through as appropriate.)Immediate Feedback & Next Steps
Same-day discussion of findings, initial strategies you can start using now, and a recommended therapy plan.
Typical length: 60–90 minutes (may vary based on needs and medical history).
Conditions We Assess
Cognitive-communication changes related to stroke/TIA, mild cognitive impairment, dementia, and Parkinson’s disease
Aphasia, apraxia, dysarthria (speech and language changes after neurological events)
Voice changes (age-related, Parkinson’s, vocal fatigue)
Swallowing concerns (dysphagia) including coughing with liquids/foods, prolonged mealtimes, or pill-swallowing difficulty
General age-related communication challenges impacting independence and quality of life
After the Evaluation

Care coordination with your physician/care team upon request
